$3,000 Diabetes Expense Grant Guidelines
The Jones Foundation has teamed up with Team Schnak Strong Fund to provide a $1,000 ($3,000 effective 2026) Diabetes Expense Grant to eligible applicants in Coffey, Lyon and Osage Counties to help offset expenses associated with the treatment of Childhood Type 1 Diabetes. This grant is payable in the amount of $1,000 per applicant, per year up to the age of 21 and may be re-applied for annually. Beginning in 2026, this grant will be payable in the amount of $3,000 per applicant, per year up to the age of 21 and may be re-applied for annually. There is no income requirement for this grant. NOTE: If you so choose, you may also apply for the Children’s Medical Expense Grant.
Eligibility Requirements for the $1,000 Diabetes Expense Grant ($3,000 effective 2026):
- Proof of Type 1 Diabetes diagnosis by either signed copy of Physician’s Letter OR from online medical portal.
- School records for Proof of Residency in Coffey, Lyon, or Osage county for the purpose of verifying that the student has continuously resided in one (or a combination) of these three counties for a minimum of one year.
- Birth Certificate for Proof of Citizenship and age (must be under the age of 21).
