$3,000 Diabetes Expense Grant Guidelines
The Jones Foundation has teamed up with Team Schnak Strong Fund to provide a $3,000 Diabetes Expense Grant to eligible applicants in Coffey, Lyon and Osage Counties to help offset expenses associated with the treatment of Childhood Type 1 Diabetes. This grant is payable in the amount of $3,000 per applicant, per year up to the age of 21 and may be re-applied for annually. There is no income requirement for this grant. NOTE: If you so choose, you may also apply for the Children’s Medical Expense Grant.
Eligibility Requirements for the $3,000 Diabetes Expense Grant:
- Proof of Type 1 Diabetes diagnosis by either a copy of signed Physician’s Letter OR printed document/screenshot from online medical portal.
- School records for proof of residency in Coffey, Lyon, or Osage county for the purpose of verifying that the student has continuously resided in one (or a combination) of these three counties for a minimum of one year. If applicant is beyond school age, please submit first page of most recent tax return verifying address.
- Birth Certificate for proof of citizenship and age verification (must be under the age of 21).
