The Jones Foundation assists families in need to cover a child’s medical, dental, or vision expenses.
Before seeking any medical treatment, a family must first contact the Jones Foundation office to complete an application to determine the needs of the family, the financial situation, the number of family members, other resources, etc. In emergency cases, it may not be possible to have a completed application prior to the emergency. In such cases, a member or representative of the family must contact the office within 15 working days of the initial treatment. The Jones Foundation reviews each application to determine whether qualifications and eligibility for financial assistance have been met. The Jones Foundation’s Board of Trustees, in its sole discretion, has the final decision regarding the qualification, eligibility, and the amount of financial assistance to be awarded.
What is covered?
The Children’s Medical Assistance program provides assistance for these services:
- General Medical Expenses
- Dental Expenses
- Medically Necessary Orthodontics (Braces)
- Eye Exams & Glasses
All authorizations for payment are made for a specified period of time or for a specific service, and no authorization is made for a period over one year at any one time unless specified otherwise. There will be no payment given for services before applying except in cases of emergency as explained above.
Who Qualifies?
To be considered for medical assistance, a child must:
- Be under the age of 21 (or under age 19 for braces)
- Be a United States Citizen and currently residing in Coffey, Lyon, or Osage County
- Have resided in Coffey, Lyon, or Osage County continuously for no less than one year immediately prior to applying for assistance
